Thank you for taking the first step towards spreading quality chiropractic care! Below, you will find the volunteer application in word document form so that you may type in your responses. There is a $25 application fee that must be accompanied with the application. If you are selected, The participation fee to attend the trip is $475 (excluding flight). For more details, please refer to the Event page.
After filling it out, you can send in the application to email@example.com, or mail a print version with the application fee.
The space is limited for the trips, so the sooner you apply the better!
